Scheduling & Appointments

New Patients

All new patient appointments get scheduled through the Patient Access Center. A physician must make the referral, by filling out the Referral Form below and faxing it to (352) 294-8054, along with all pertinent medical records. All appointments are made for the first available opening. Once an appointment is made, the Patient Access Center will mail out a questionnaire that should be filled out in advance and brought to the initial appointment. In the event that the child cannot wait until that appointment, the referring physician must call (352) 273-9350, to speak with the doctor on call to discuss the situation.


Return Patients

Patients can make return appointments by calling our scheduling line at (352) 273-9350, or click the Request An Appointment button below. Return appointments can be made, so long as the patient has been seen by us in the last 3 years

All pediatric liver transplant patients must schedule through the transplant department by calling (352) 265-3288.


Procedure Scheduling

Procedures are scheduled by the nurses, usually before the child leaves clinic. In the event that the appointment is not scheduled prior to the child leaving the clinic, the nurse will contact the parents to schedule a date and give all the necessary information. Test Scheduling

Tests are scheduled by the nurses and clinic staff through the department that performs the test. Since the tests have to go through other departments to get scheduled it may take several days to get completed. Once the details are finalized, either the GI staff or the testing department’s staff will call to inform you of the necessary information.
